Memorial/Honorarium

A memorial gift to Community Hospice Foundation is a thoughtful way to remember and honor someone you love. Your donation helps support the patients, families and programs of Community Hospice.

Named Gift Opportunities

Named gifts are a wonderful way to honor a loved one. Naming opportunities are available at both the Community Hospice Alexander Cohen Hospice House and the Haig & Isabel Berberian Patient Services Center. Opportunities ranging from $2,000 to $250,000 are available. Pledges over $7,500 may be paid over 3 years. A plaque naming the item or area along with a private dedication is provided.

Veterans Pavilion

Community Hospice is a proud partner of We Honor Veterans, a program of the National Hospice and Palliative Care Organization, which certifies Community Hospice professionals to honor the unique needs of veterans and their families with specialized patient care and grief support. Opening in 2016, the Veterans Pavilion at the Haig & Isabel Berberian Patient Services Center will give families of veterans the chance to honor their loved one with personalized pavers inscribed with name, rank and branch of service with a gift of $1,500. A donation of $100,000 or more entitles a donor to permanent recognition of the pavilion.

Garden Grotto Wall

Many families who have passed through the doors of the Community Hospice Alexander Cohen Hospice House find comfort in creating a legacy for their loved one on our Garden Grotto Wall. Donors who contribute $2,000 or more can add a name to the Garden Grotto Wall.

Memorial Bricks

A brick placed under our beautiful rose arbor is a special way donors can memorialize or celebrate the lives of their loved ones at the Community Hospice Alexander Cohen Hospice House. A gift of $500 will be acknowledged with a personalized brick that will be installed and you will be presented with a replica brick for display at your home or office. Donors will also be invited to a Brick Dedication Ceremony to give your loved one special recognition.

Community Hospice Giving Tree

A locally handcrafted piece of wall art in the shape of a tree will come to life in our Haig & Isabel Berberian Patient Services Center lobby. Featuring green, gold and silver metal heart shaped leaves engraved with the donor’s name or name of a loved one; this is an inspirational way to memorialize a loved one. The tree will be dedicated in 2017.
