Memorial/Honorarium

A memorial gift to Community Hospice & Health Services Foundation is a thoughtful way to remember and honor someone you love. Your donation helps support the patients, families and programs of Community Hospice & Health Services.

Memorial Bricks and Pavers

Purchasing a memorial brick or paver provides an opportunity to honor a loved one and allows us to continue to provide care for our patients and families when needed. Memorial Bricks are available at the Alexander Cohen Hospice House as well as at the Veterans Pavilion at the Haig & Isabel Berberian Patient Services Center. Purchasing a paver proves an opportunity for community members to honor a loved one. Community Hospice & Health Services is a partner in the “We Honor Veterans” program, which is a collaboration between the National Hospice and Palliative Care organization and the Department of Veteran Affairs.

 

Community Hospice & Health Services Giving Tree

The Community Hospice & Health Services Giving Tree is a unique artful masterpiece created by metal designer Jose Caballero. Purchasing a leaf or branch on the Giving Tree provides an opportunity for patient families, donors and community members to honor or memorialize a loved one.

Named Gift Opportunities

Named gifts are a wonderful way to honor a loved one. Naming opportunities are available at both the Community Hospice Alexander Cohen Hospice House and the Haig & Isabel Berberian Patient Services Center. Opportunities ranging from $2,000 to $250,000 are available. Pledges over $7,500 may be paid over 3 years. A plaque naming the item or area along with a private dedication is provided.
